I've seen things like this when two databases having the identical DBID are on the same server, but nothing approaching a situation where one database is directed to handling the wrong DBID. I presume there might be such an issue with redirected databases, I can't think of much else that'd cause it, offhand.
The server itself manages locating the database by replica ID, so it'd have to be handed either a memory-overwrite or a database with something really odd, to mess up something as basic as the DBID.
